What Actually Happens in Your First Therapy Session – No Surprises

For a lot of people, the hardest part of starting therapy isn't finding someone. It's the uncertainty of what happens once they get there. The not knowing what to say, whether they'll cry, whether the therapist will judge them, whether they'll have to talk about things they're not ready to talk about yet.

I'm Ashley Partin, a professional counselor at Life Success Counseling in Cincinnati. I want to walk you through exactly what a first session looks like — because most of the anxiety around starting therapy comes from not knowing what to expect, and that's entirely fixable.

Before the Session Even Starts

At Life Success Counseling, all sessions happen via telehealth — which means your first session takes place wherever you feel most comfortable. Your living room. Your car during a lunch break. A quiet space in your home. You log in at the scheduled time, and we start from there.

There's no paperwork to fill out in a waiting room. No awkward small talk with a receptionist. Just you, a secure video connection, and a therapist ready to listen.

What the First Session Is Actually For

A first session is not about fixing anything. It's about understanding.

My job in that first hour is to get a clear picture of who you are, what's been happening, and what you're hoping therapy might help you with. I ask questions — not in a clinical interrogation way, but in the way a curious, careful person would ask when they genuinely want to understand your experience.

You don't have to have it figured out before you come in. Most people don't. "I just feel off, and I can't explain it" is a completely valid starting point. So is "I don't even know what I need — I just know something isn't working."

You're also not obligated to share everything in session one. Therapy builds over time. What you share, when you share it, and how deep we go is always your call.

What I'm Listening For

While you're talking, I'm paying attention to patterns — not just the content of what you're saying, but how you talk about it. What feels heavy. What you gloss over. What seems connected that you haven't connected yet.

I'm also assessing fit. A first session is as much for you to evaluate me as it is for me to understand you. Does this feel like someone I can trust? Does this feel like a space where I can be honest? Those are the questions worth paying attention to.

What Happens After

At the end of the first session, we'll talk about what comes next — whether that's scheduling regular weekly sessions, what focus areas make the most sense to start with, and any questions you have about the process.

The mental health therapy session isn't a commitment you make blindly after one session. It's a decision you make with information. The first session is where you get that information.
